Specimen Collection Criteria

Collect: 2.0 mL whole blood into a special tube provided in the Burroughs Wellcome Thrombo-Wellco test kit. Send to coagulation department immediately.

Each tube contains Bothrops venom to ensure complete clotting of fibrinogen and a fibrinolytic inhibitor to prevent breakdown of fibrin in vitro.

Physician Office/Draw Specimen Preparation

Allow specimen to clot for 30-60 minutes at room temperature (20-26°C or 68-78.8°F). Centrifuge to separate serum from cells immediately after clotting. Transfer serum into plastic transport tube and freeze (-20°C/-4°F or below). Place specimen on DRY ICE prior to transport. Specimens must be received in the Laboratory within 6 hours of collection.

Preparation for Courier Transport

Transport: Serum in plastic transport tube, frozen (-20°C/-4°F or below).

Rejection Criteria

Specimens that are not collected in the special Thrombo-Wellco tubes.

Specimens containing less than 0.5 mL.

In-Lab Processing

Allow specimen to clot for 30-60 minutes at room temperature (20-26°C or 68-78.8°F). Centrifuge to separate serum from cells immediately after clotting. Transfer serum into plastic transport tube and freeze (-20°C/-4°F or below) prior to testing.

Storage

Specimen Stability for Testing:

Room Temperature (20-26°C or 68-78.8°F): 6 hours
Refrigerated (2-8°C or 36-46°F): Unacceptable
Frozen (-20°C/-4°F or below): Serum aliquot must be frozen for transport.

Specimen Storage in Department Prior to Disposal:

Room Temperature (20-26C or 68-78.8F): 7 days
Refrigerated (2-8°C or 36-46°F): 7 days

Performed

Sunday – Saturday, 24 hours a day.
STAT results available within 30 minutes of receipt in the Laboratory.
Routine results available within 4 hours of receipt in the Laboratory.

Reference Range

Normal Value: Less than 10 mcg/mL.

Test Methodology

Latex Agglutination (Thrombo-Wellco).

Interpretation

  • Fibrin split products are elevated in DIC.
  • This test is not specific for Fragments D and E but will also react with fibrinogen, FSP (X, Y, D and E) and fibrin monomer.

Clinical Utility

  • Fibrin(ogen) split products are formed by digestion of fibrin(ogen) as a result of activation of the fibrinolytic system.
  • This test aids in the detection of DIC.
  • This test is not to be used for the exclusion of deep vein thrombosis (DVT) or pulmonary embolism (PE).
